The programme is not just dedicated to those suffering from chronic diseases such as diabetes, but it is also aimed to prevent the development of health risks. The Medcare Disease Management Programme is positioned to be a gateway to living healthier, complete with tools to improve health status and support patients in making positive lifestyle changes.

According to recent global statistics, disease management programmes have shown significant improvements in the lives of patients. For instance, in Germany, programmes focused on type 2 diabetes noted a reduced incidence in complications, followed by a decrease in overall care cost by 13 per cent, leading to increased patient satisfaction with treatment.

Healthcare facilities in Japan recorded weight loss, which in turn reduced the risk of diabetes by two-thirds among patients with impaired glucose tolerance. Other countries recorded improved survival rates, making disease management programmes an ideal guide for UAE residents.   

At Medcare, a treating physician will create a customised medical care plan made to suit patients' specific conditions, which will include required specialists visits for the upcoming 12 months, as well as set health outcome targets to be reached within 12 months.

Patients will also be assigned a personal case manager, who will regularly interact with the patient regarding results and targets. The case manager will also have access to your care plan to schedule appointments, send reminders and follow up accordingly.
